NEW! In 2016 is an 8.5×11” format for the TLR standard logos. If you choose to print on colored bond, the colors are indicated below.  Each is printable and can also be filled in electronically:

Independent Mobility (print on ivory bond)
Guided Mobility (print on blue bond)
Standing Transfer with Belt - One Worker (print on goldenrod)
Standing Transfer with Belt - Two Workers (print on yellow bond)
Sit/stand Lift (print on green bond)
Total Lift (print on red bond)
Reposition (print on white bond)
Bed Rest (print on pink bond)
