The TLR Instructor Qualification Assessment document is provided in Word Format. Current content is not to be removed. This is the minimum to be assessed by SASWH approved TLR Instructors working to successfully qualify co-workers to the TLR instructor level. Prior to printing you may want to add some space for the areas where you would provide written comments.

Downloadable and printable documents (no charge) will continue to remain on the program site.
