This form is used when a PART® Trainer is qualifying to be a PART® Instructor.
Current qualified PART® instructors co-teach/observe Train the Trainer sessions with instructor candidates, a minimum of two times*, with a final co-teach/observation being facilitated by a SASWH Instructor. Please submit the below form, filled out with all co-teach/observation records, to SASWH when requesting the final co-teach/observation.
